California Privacy Act Policy

Effective Date: June 25, 2026

This California Privacy Act Policy (“CA Privacy Policy”) supplements Belgrave Family Dental Center’s Privacy Policy and explains the rights of California residents under the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA).

1. Scope

This policy applies to California residents whose personal information we collect through our website, online forms, in-person visits, or other communications. Information governed by HIPAA and the CMIA is handled under our Privacy Policy & Notice of Privacy Practices.

2. Categories of Personal Information Collected

In the preceding 12 months, we may have collected the following categories of personal information:

  • Identifiers: Name, address, phone number, email
  • Commercial Information: Appointment details and services booked
  • Health/Medical Information: Treatment preferences or notes (voluntarily provided)
  • Internet Activity: IP address, browser data, and interactions with the site collected via tools such as Google Analytics and Cloudflare
  • Inferences: Preferences or interests based on treatments or inquiries

3. Sources of Information

We collect this information directly from you and from forms on our website, email and phone communications, cookies and analytics tools, and social-media interactions.

4. Use of Personal Information

We use this information to schedule and manage appointments, recommend treatments, communicate with you, improve our services, and comply with legal obligations.

5. Disclosure of Personal Information

We may disclose your information to authorized service providers (such as schedulers and email or marketing platforms), to legal authorities when required, and to third parties with your explicit consent.

We do not sell your personal information.

EDITOR’S NOTE (remove before publishing): Under CPRA, passing identifiers to Google or advertising tools can sometimes count as “sharing” for cross-context behavioral advertising even when no money changes hands. Whether the current Google tag setup triggers this should be confirmed by the attorney; if so, a “Do Not Sell or Share My Personal Information” link may be required.

6. Your California Privacy Rights

If you are a California resident, you have the right to:

  • Know what personal information we collect and how we use it
  • Request deletion of your personal information
  • Correct inaccurate personal information
  • Opt out of the sale or sharing of personal information (if applicable)
  • Limit the use of sensitive personal information, including health information
  • Be free from discrimination for exercising these rights

7. How to Exercise Your Rights

You may submit a request by email or phone using the contact details below. We may take steps to verify your identity before processing your request, and you may use an authorized agent to submit a request on your behalf.

8. Data Retention

We retain personal information for as long as needed for business purposes or legal compliance, depending on treatment type, patient engagement, and regulatory requirements.

9. Do Not Sell or Share My Information

Belgrave Family Dental Center does not sell or share personal information as those terms are defined under the CCPA/CPRA. If this changes, we will update this policy and provide a method to opt out.

10. Updates

We may revise this CA Privacy Policy periodically. Please check this page for the latest version, indicated by the effective date above.
